Application Forms

There are several application forms that seafish use and request from fishermen.

They are used to obtain access to the following items

  • To obtain a photo ID Card or "ticket" 
  • To obtain grant funding
  • To obtain a free Personal Floatation Device (PFD)

An explanation of each of these categories along with a link to download the specific application form is outlined below.   

  

 

New Entrants Photo ID Card 

Following the completion of all 4 basic courses

  • Sea Survival
  • Fire Fighting
  • First Aid
  • Health & Safety

New entrants can apply for their 4 in 1 card which is a credit card sized document listing the 4 certificates obtained. To apply for this card please download the application form here.

 

Experienced Fisherman`s Photo ID Card 

This card is obtained following the completion of all 4 basic course plus the

  • Safety Awareness course.

Experienced Fishermen can apply for their 5 in 1 card which is a credit card sized document listing the 5 certificates obtained. To apply for this card please download the application form here.

 

Under 16.5m Skippers Certificate Photo ID Card (Silver and Gold categories) 

These cards can be obtained by experienced fishermen who have already completed the 5 courses above and who have then gone on to complete the following set of courses.

  • A 1Day Stability Awareness Course.
  • A 1Day GMDSS Short Range
  • A 2Day Bridge Watch-Keeping Course
  • A 2Day Engineering Course

Once this set of courses have been completed fishermen can then apply for their Under 16.5m (Silver) Skippers Certificate. Completing the courses above entitles a skipper to take control of a fishing vessel up to 20 miles off-shore. This Certificate is also know as a Restricted Ticket. 

For fishermen wishing to take control of vessels beyond 20 miles off-shore 2 additional courses need to be completed.

  • A 5Day Bridge Watch-Keeping Course
  • A 5Day Engineering Course (MCA 30 Hour Advanced Engineering Course)

Once these 2 additional course have been completed fishermen can then apply for their Under 16.5m (Gold) Skippers Certificate.   This Certificate is also known as an Un-Restricted Ticket.
